The role of the Clinical Nurse is to provide and coordinate resident care, and to coach and mentor  staff to ensure an effective, efficient functioning team. The Clinical Nurse will perform in accordance with legislation and practice within a professional and ethical framework. The CCC is responsible for assessing and evaluating individual resident care at a systems level to ensure service delivery reflects evidence based best Practice. This supportive role is to work cohesively with the Executive Director and Care Director on clinical service delivery, compliance and audit process, and to act as a clinical resource for staff, where effective leadership and clinical education will feature as a major component of the role.

Professional Practice 

  • Ensure that care is delivered in accordance with the philosophy, policies and procedures of Estia and complies with all relevant local, state and federal legislative requirements and the Commonwealth Government Aged Care Act 1997 and related principles.
  • Lead and participate in the delivery of a high standard of person centred care.
  • Provide a high standard of ongoing clinical management and performs nursing interventions in accordance with recognised standards of practice.
  • Assist and Educate staff to complete comprehensive, individual assessments and care plans for new and existing residents, and evaluate care in line with Estia’s policy.
  • Ensure that all clinical documentation is completed accurately and in a timely manner in accordance with Estia’s policies and procedures. These include but not limited to: care plans, ACFI, clinical assessment, resident of the day program.
  • Ensure the social, emotional spiritual and recreational needs of all the residents are a high priority in the care regime and that all care staff are supported in working towards these needs.
  • Ensure that families and significant others are included in the care planning process and in any decisions affecting the resident.
  • Practices in a way that acknowledges the dignity, culture, values beliefs and rights of individual residents are respected.

  
Coordination of Care

  • Ensure that staff comply with all requirements for Compulsory Reporting of Abuse.
  • Ensure that all hazards and incidents are correctly documented, acted upon and reported in line with Estia’s Policy.
  • Comply with the Requirements of the National Safety & Quality Health Service Standards
  • Comply with all relevant clinical and/or competency standards
  • Comply with the principles of Patient and Family Centred Care that relate to this position
  • Take reasonable care to maintain their own health and safety and that of those around them
  • Demonstrate conduct to ensure a safe and healthy environment is maintained at all times
  • Make themselves aware of the correct policies, processes and procedures to maintain health and safety and communicate to others
  • Actively participate in making Estia Health a safe and healthy workplace through communication and consultation processes.

KEY SELECTION CRITERIA

  • Must be a registered nurse
  • Strong interpersonal and communication skills both written and verbal
  • Demonstrated ability to work within a multidisciplinary team and autonomously
  • Proficient computer skills
  • Demonstrated ability to effectively liaise with external service providers/ stakeholders
  • Effective time management and organisational skills
  • Ability to work well under pressure and be flexible to changing priorities and environment
  • Commitment to quality and a broad knowledge of all facets of contemporary aged care including ACFI funding and Accreditation
  • Knowledge and understanding with implementation and management of Workplace Health & Safety
  • Program / Infection Control, Manual Handling
  • Demonstrated understanding of the relevant Laws, Acts, Regulations, Charters and Codes of Practice
  • All preferred candidates will be required to undertake a National Criminal Record Check
